I require PDF form designed for our agency to display our fees and also compare other agencies fees. The form will later be converted to an interactive form to calculate fees based on custom fields, but this initial competition is for the design/layout only. Once we get the design right, I will have someone convert it to be interactive and display calculations etc.

I am quite flexible with the layout and design, but it is important that the form has a nice professional look and is easy to understand for the user (it will be used as a marketing tool when we try and acquire new business).

We are a real estate agency named About property Management and we manage rental properties for our landlords throughout Brisbane Australia. Our website is http://aboutpropertymanagement.com.au. I would like the form to match our current branding and colour scheme etc.

I have attached our logo file to use

I have also attached a document named PDF2 which shows all the information I would like on the PDF.

Thanks everyone. Some nice looking designs so far.

I am noticing the designs are looking a little cluttered with so many columns. I am open to reducing the number of columns for anyone that is still in the process of making new designs.

The second last column (Typical Real Estate) can be be removed if it helps with the design. This will allow more room to make the first column (About Property Management) more noticeable.

The first column is our agencies fees and needs to be more prominent. The three other columns are pretend agencies and should be less prominent (they are just there as an example).

The 'About Property Management' and 'Enter Custom fees' columns are the sections that should draw the most attention.
