I need a paper-card insert designed for a product i am selling. The papercard is 105x110mm. I have attached examples of what it looks like.

I need the papercard to have my company logo, which i attached as "Muddboxx", my company name, and needs to say the product name which is "Foot Cushion & Forefoot Gel Pad".

Only put the following images on the design: Transparent Background, Ball of foot cushion, Forefoot

DO NOT PUT Screen Shot 2018-04-20, or IMG_20042018_184243_0 (1) on the design!

This is a pretty simple design needed so please respond asap. I have logo other file types if needed
