Please see attached jpg for photo of existing template for reference.

Description - I am about to run out of sheets for my closest-to-the-pin hole prizer markers. I need to get more printed and want to use this opportunity to design something better than what I have now.

Dimensions - 4.75" wide by 7.5" tall

Logo - Would love to include our league logo (artwork attached below) as part of the template.

Series of blank lines for people to put their name if they win

Notes/Misc - Notice that the numbers in front of each line on the existing template are not visible when the template is slid into the marker. Line numbers aren't a must but if you do include them in your layout please offset them a little more to right so that they are visible.

Web site - I would also like to put "www.loutour.com" at the bottom of the template, preferably centered.

Doesn't have to be black and white or boring. Feel free to be as creative as you want so long as it is still functional.

I am going to buy at least two versions. Hector B for sure since he has endured and kindly made at least five or six rounds of revisions to his concept and also Md Saiful H. (great job). Awarding prize now. Pretty sure once I pick one winner I am also able to buy a second one too. Please bear with me while I figure out how to do that.
