Took a quick look at the first tax notice for tax year 2017. Here are my immediate observations:

1. 'Synapse Media' 1099'd you for $3,874. Your Accountant did not report this income. (This does not have a big tax effect.)
2. On income reported by your accountant, your accountant neglected to report (sic pay) $7,174 of Social Security tax owed. (This one does have a big effect!)
While we can capably mitigate the "Synapse Media" issue, the Social Security tax is hugely problematic and not easily fixed. I STRONGLY RECOMMEND YOU GET THIS PAID, as the IRS is hitting you with penalties / interest for every day you do not.
I have not reviewed your other documents yet. But, if the other years are as reflective of your tax position then we have immediate and appropriate work to do!
