Build forms into my existing website

1--I have an existing website using weebly and I currently use Bookeo for my online booking. I am looking to add to it a form for hiring contractors. The will need to have the ability to have the applicants upload documents and pictures, and also an online form with medical health questions, and also a diagram where the client can indicate their problem areas, and room for explanations. I would like to be able to access this from my phone so that I can easily provide this form to the contractors on site. I would like all of the features to be easily accessible from both desktop and mobile sites. 2--As part of this application process, I also need to link a business contract (no compete) for which I can provide the verbiage. I would like to have it in electronic form to be signed electronically by new contractor applicants. I would like to have it automatically sent to the applicant via email for their records, with the format being one that is easy to print and view on both desktop and mobile devices. 3--I also will need to have an electronic form that can be completed by my clients during the online booking process, and that I can direct people to (so in the event that they call to book, I can schedule the appointment and refer them to the intake form or send a link). 4--I would like to integrate a zip-code based gas-surcharge calculator into the existing check out process, so that when a zip code is entered, it adds a predetermined fuel surcharge to the price before payment (excluding those who buy prepay packages) 5--I would like to have an app built for the contractors that I hire so they can access and accept potential jobs that I post there and view all the forms for employment. I would also like a basic invoice generator so that contractors and invoice directly from the app, and get a copy sent to their email. And I would like the financial information of AR/AP to populated for my accounting needs. I don't know if this is one big project, or several small ones, but I have listed everything in order of importance to me. Please let me know what other information you need.
