Build an online form on Joomla

UNDERSTANDING: [login to view URL] want us to develop Joomla website into which we need to implement two (immigration)forms which authorized users would be able to fill up. [login to view URL] credentials will be generated by the admin using which users are able to login and choose the one the form suitable for them. Please confirm that the form should be on webpage or in the pdf (pdf will have fields where user can fill data).? also is it mandatory to go with joomla cms? because the requirment is complex so prefer to go with custom code. the attached two forms are not fillable in pdf so i presume there will be web forms. Do u want to merge these two forms? when sending?
