Contact Us – Page - Form *Module for Map locations already installed *Contact us form is the Magento 2 default *Form must be able to email 1 of 4 different email addresses from the desired location selection *Layout will stay the same as it already is. Items for the Contact Page. *All fields need to have a clear visible box (maybe make the field more grey), add black ring around the field, like date and time fields. Map visible of all 4 locations prior to selection. (This is already Setup) Ability to add more locations, 4 locations currently: listed with address, phone number, and email address. (This is already setup.) Once location is selected zooms in on appropriate location & Contact message will route to the selected locations email address. (This is already setup) Contact us has a drop down with desired location to contact. (South, North, etc..) (This is already setup, but not linked to the email addresses required) Second, drop down with the ability to select a Subject. (Have A Questions, Get A Quote, Make An Appointment, Other) This subject will be used in the title of the form email. When selecting make an appointment The field selection for Date and Time will then appear to select. Mon-Fri 8am – 5pm ***this is not going to make an appointment, it will only send the date and time information via email to the desired location email address.1 of the 4 selected email addresses*** Contact fields – *Select Location,* Name, *Phone number, *Email address, *Subject (have a question, request a quote, make an appointments (drop down w date and time with this option), other), *message. * are required fields.
