I would like an IOS app developed which can be used on an iPad with the following functionality:

The app will be a form with 4 fields, 3 of the fields will be populated by options retrieved using a REST api call (1 call for each field), the api returns data in json format, I can supply sample api calls in php format, they are simple GET calls. The 4th field is a manually entered number.

The submit button will use the form data to create an api call, again in json format.

I would like the field options to be displayed as tiles, similar to Windows 10.

The end product needs to be delivered in a format that allows me to edit the app in the future.

The actual purpose of the app is to enter vehicle fuel data.

Field 1 - Select a location (with a default option)

Field 2 - Select a person

Field 3 - Select a vehicle

Field 4 - Fuel amount

I forgot to add there will be a requirement for a settings function within the app so that the user can add their username and password, this data is used to create the api calls.
