The following site corrections and modifications are requested. 1. When using the Mobile version of [login to view URL], The menu says MOD_MENU. It should only say: MENU next to the "hamburger icon" 2. When viewing on a smartphone, the appointment form does not display correctly on: [login to view URL] Field names overlap the data entry fields. (See attached screenshots) 3. The Contact Form needs repair/modifications on [login to view URL] a. Tab order is out of sync on two fields (City and State). When you tab thru the form, it goes to State after Street Address, needs to go to City first. then State, then ZIP Code b. Only the following fields should be required (and should have asterisks noting they are required) First Name* Last Name* Street Address* City* State* ZIP* Email Address* All other fields should be changed to optional (they are currently required) c. Submit button should be underneath Captcha and use a colored button d. Upon successful submission, form needs to return a Verification page: Thank you for contacting us, "First Name". We will get back to you as quickly as possible. (Usually within a couple hours on Weekdays. If submitted on a weekend, we'll get back to you on Monday morning.)
