Online course booking form

Brief At present my client has a paper form which they have to complete whilst taking calls from clients wanting to book Health & Safety courses with them. The aim is to move the form online and have clients complete it themselves. The data from the form is to be collected in a spreadsheet and used: (1) send out confirmation letters to the client; (2) print out the Certificates; (3) create pass cards. I have not included this below as I am not sure how practical this would be if the data has to be emailed. I am also pretty sure the client would not have a picture of every delegate they want to send on the course at time of booking. The form is in 2 main sections: 1). Main Part of Form Company, Name (required), Address_1 (required) , Address_2, Address_3, Town (required), County (required), Postcode (required), Account No., Order No., Contact Person (required), Landline (required), Mobile (required), Fax No., Email (required), Course Name (required), Course Date (required), Training Location (required), No. of Delegates (required). 2). Delegates Form section (max number of delegates is 20) Delegate First Name (required), Delegate Surname (required), Delegate Age (required), Delegate Experience – No. Years:Months (required), Card Holder Requirement • To enable the user to add Delegates up to the limit set by their selection in the No. of Delegates drop down. • To validate form data entry or missing required fields • To provide a script to process the form data and email the completed form data (tab delimited) to a recipient so that it can be copy and pasted into a spreadsheet. For end user simplicity this means repeating the ‘Main Part of Form’ for each person added to the ‘Delegates Form Section’ Each row paragraph separated. Preferred Techonologies: html, php, css, ajax
