Community Builder Configuration - Joomla Website

We would like a contact form to be added to our website which will either be community builder or work with and prepopulate community builder. We have community builder installed on our site already and we require all of the fields in the contact form created by you to pre-populate community builder fields. We only wish the administrator to have access to the information posted on the contact form and not every registered user. We should be able to decide which information is shared following submission by the registering user. The contact form should request the following details from the user: Title - (Mr, Mrs, Miss, Dr) First Name (Text field) Last Name (Text field) First Line Of Address (Text Field) (NOT ESSENTIAL FOR SUBMISSION) City (Text Field) Country (Text Field) Post Code (Text Field) Phone Number (Text field) Email Address (Text field) Web Address (NOT ESSENTIAL FOR SUBMISSION) Enquiry (Text field box) **All of the above fields are mandatory apart from the ones marked not essential. ** We require that all of the above fields are completed by the user and when they send the information to us they are sent a well designed (including an image and well designed layout) confirmation email message confirming receipt of their enquiry. We also require a well designed email through to the administrator email account/s specified for them to receive and contact the user regarding their enquiry. At the bottom of every article on our site we would like a sentence added to say: To find out more about our services please click here to "Contact us". The "Contact us" in the above sentence will link to the contact form you have created. We require this project to be completed as soon as possible. This project is not large and should not take you long however it is fairly urgent.
