Add two products to existing PHP order form

Hello, I need you to add a small box to my existing order form so the visitor can change the amount he wants to purchase. The only traffic plan available in my existing order form is the one at 9000 visits. I need you to 50,000 and 100,000 visits plans as displayed in the attached screenshot. The order form is currently coded to show a price of $12.95 when there is a US STATE chosen and $18.95 when there is a city chosen. Plus it adds $5.90 for the plan of 9000 visits when the 1 POPUP checkbox is checked. I need you to increase those values when the 50,000 or 100,000 options are chosen. The total price should be increased by 100% when city is chosen. Currently it adds only $12 to the price of $12.95 so $24.95 when city is chosen. Also if the 1-popup option is chosen the price should be increased by another 50%. So if both options are chosen, the price should be increased by 150%. Sample: 50,000 visits with city targeting and 1-popup option goes from $49.50 as displayed on screenshot to $123.75 if only city targeting is chosen for 50,000 visits then the price would be $99 (+100%) Thanks Note: This script has NO database connection of any kind.
