Build an online form website

Problem description: We have a number of users and we want them to submit a from on a monthly basis ( 1 form per month), this form will be printed by the admin or exported to excel There are two roles , admin and user Function Requirements.: 1- Admin panel (cms) to set some general settings ( date range to allow the data entry for the user i.e for jan. form I want it be from 25 jan until 1 Feb. and so on) the dates are specified by the admin 2- For the admin , he can use the panel to set some specific code that will allow the users to register (I don't want anyway without the code to register in the website) 3- For the admin , he can search through users and see the forms , print or export to excel (form is in arabic and I can assist in this) , CRUD operations for users 4- For the user , on that specific date he can login to the system , enter the data into the form , and maybe edit that data on the specified date range but after that period ends he cannot do anything 5- There should be some messaging between the admin and the user in case the user have made some error in the entry or have some question Modules: 1- Admin panel (cms) 2- Membership 3- Messaging similar to a simple ticket system Note: The source code is give to me after the system is completed.
