I have form (Ranges) that sends a Json array to MySQL db.
This form contains several sections: Range, Condition and Reference.
I use Ajax Select2 and MySQL to populate the drop down lists in the form.
Most fields in Condition section are hidden (display:none) and will be shown based on selection of drop down list using Ajax.
What I need, write a JQuery script that creates a new row with form fields for Conditions. Every Range can have multiple Conditions by a button click Write a JQuery script that creates a new row with form fields for References. Every Condition can have multiple References by a button click.
Also, the new created rows needs also the option be removed if necessary.
If new condition section is created by button click, a tab or button needs to be placed above the condition section to call and see the previous conditions, and see the corresponding references.

I have no access to my webserver so I send you necessary files.
