HTML5 Smiley feedback form

Project Description Smiley face feedback form that runs through the browser. Needs to use HTML5 / PHP / MySQL and captured the date/time of the click and a comment if used within the webform. I have included an image to show you what is required. I would like to open this on an android/apple device (tablet) and keep it running all day. When people walk by they click the smiley and have the opportunity to capture a comment (like the image attached). The feedback form must refresh/reset once someone has clicked on a smiley. I will be running this on a cpanel server and need by tomorrow evening - I know this is a very quick project for someone with the right skills. Thanks
