Inquiry form / question system for WordPress ( with load screen )

Hey Freelancers! We need the function on our WordPress page to insert and edit a question system like on the following link: [login to view URL] feed-in%22%7D%5D Here you can click on the respective questions to get to the next one. The last question is then the query of the postal code ( PLZ ) with the Germany map. It follows the loading screen, in which "suitable companies are searched" ( this is of course only an effect ). Thereupon comes the message that in the proximity suitable companies were found and the form for contact with "Mr" "Mrs" - First name - Last name - Telephone number - E-mail address and acceptance of the privacy policy. Below the button "Receive offers now" - The request must then go to our e-mail address. Our requirements: - If possible, it should be a WordPress plugin in which we can set and edit the questions ourselves. - We must be able to adapt and insert the images ourselves. - There must be a different number of questions possible ( 3 - 5 ) - It must be possible to have more than one answer to a question and to tick them. If possible, it should be as close as possible to the link sent above. A simple request form with several questions, the postcode input and the load screen, after which follows the indication of the contact data which will be sent to us by mail. Of course, everything should be in German. Also the notifications if e.g. the request was sent successfully. Of course we provide the texts and the content. In addition we attach great importance to the fact that the inquiry form is Responsive and also on mobile devices looks and functions perfectly. Looking Forward to your offers!
