JWT(JSON Web Token) login/logout page by php/javascript/mysql/metamask

Hello, This project is for creating a login/logout page by php/javascript/mysql/metamask. It is not a normal email/password login/logout page and it needs metamask integration. Please check [login to view URL] for sample. It may use JWT(JSON Web Token). another sample for personal sign. [login to view URL] Please implement by php for the server side. Requirements: 1) siginin page consists of address , email , username like [login to view URL] all the login by ethereum address functionality. save email and username with mysql. 2) If the user's metamask is locked, the signin page will show some descriptions about installing metamask and login to the metamask etc. If the user's metamask is unlocked, the signin page will show address , email form , username form. web3 feature may be required for the integration. verifying whether the address is right for the user or not is required. the user will sign like [login to view URL] amd address,email, username have to be saved in mysql. user can login by the metamask sign. 3) when the user logout in the metamask, the user has to logout the page too. 4) secure code with comment. 5) one configuration file if possible. switch test and production environment. 6) the design of UI can be basic. responsive. 7) it should show right messages for user input. error message etc. For more details, please contact me.
