Registration form with Credit Card Processing and Admin Panel

We want to redo our current form: [login to view URL] It is a basic Registration Form, with radio buttons, drop down menus, and Credit Card processing using our Quickbooks Merchant Account (not paypal, REAL credit card processing) PLEASE READ!! The form will have the same information it has in the link above. Student will list their name, email, contact, and select their type of class. The type of class will have different payment amount and different choices to select. The form will do the following: 1. Contain Captcha to block spam 2. Show a Registration Complete page after credit card as been processed. 3. Will send the student an email confirmation 4. Will send us a confirmation 5. Student will be entered into database, and Admin Panel 6. Admin Panel will show all registered students 7. Admin panel will allow me to Edit the Registration, the dates, etc. 8. The admin panel will allow me to send the student a confirmation after I edit or change their class or dates 9. I will be able to send a confirmation to the Instructors as well I want to see forms you have created. No BEGINNERS. If you do not have completed working forms to show me, we are not interested. Please do not send us your websites to look at. We only want to see forms that you have created. If this is missing, we will delete your proposal.
