Simple Modifications to PHP Script

I have detailed this simple project VERY THOROUGHLY, so even though it looks like alot, it actually isnt. I detailed this to eliminate the need for many questions. I have a couple of modifications to a php project. The php project script files enable a user to go to our website and request a song to play on our radio station. Currently there are a couple of things that I dont like about it. 1) Its not mobile friendly 2) Its not graphically appealing 3) When searching, it doesnt display the list how i would like it. All the changes you are doing are COSMETIC with the exception of one functional change. I have attached the project files to this project, please have a look at them before you bid. Additionally I would like this project done right now. It should only take an hour or so, so if youre available right now to do this simple project then pleas ping me. Heres the changes that need made: Add our logo to the top of the page Make it look mobile friendly Make the search box so that when the user types in the name of the artist or title, it populates a list as they are typing. Heres the flow: User enters the [login to view URL] page and sees 2 options (two cells that are different color background) The two options are Search by Artist & Search by Title Then when they make their selection they see a search box, then submit When they start typing in the search box it starts showing results in a dropdown list and when they see the one they want, they select it, then click submit. The logo will remain at the top of all the views that the user will see. So the user will always see the logo no matter which page they appear to be on. You may add a "includes header" file if this makes it easier for you.
