Create Form with PowerApp using PowerAutomate for approval workflows on O365

Transition SharePoint Solution to Production Form I, people picker field, confirm all pre-production testing has been completed by me or my designated testing team and we have found the solution text field for name of site/app/form to be free from errors or bugs. (Both fields are Required) The site owner will be people picker field. This owner has confirmed they completed all required SharePoint training. (Required) The site sub-admins/full control users will be, people picker field, people picker field, people picker field. I certify each of these users has completed all the required SharePoint training. (All three are Optional) Please promote the solution from (development or test – CHOICE DROP DOWN) on calendar date picker to production (CHOICE DROP DOWN). (Required) PLACE TO CAPTURE SUBMITTER SIGNATURE OR CAPTURES APPROVAL BY SUBMITTER (Required) My supervisor, people picker field, has approved the production promotion date above. My supervisor has also approved the site owner, and if designated, the users listed above for full control. (Required) ---Form routed here via email to Supervisor for Signature/Approval (Email should have the information above in it and include the submitter name.) PLACE TO CAPTURE SUPERVISOR SIGNATURE OR CAPTURES APPROVAL/REJECTION BY SUPERVISOR (Required) ---After signed/approved by Supervisor, the form is routed via email to the SharePoint Admin Group for approval. PLACE TO CAPTURE SHAREPOINT ADMIN APPROVAL/REJECTION (Required) If rejected, the SharePoint Admin must be able to put in comments that will be included in the email back to the submitter. After SP Admin approval/rejection, the form is final, email goes back to submitter indicating the decision and comments, if rejected.
