As it is shown there, requirement is simple and I can do this with standard .NET forms. However, as you mention there could be future work, and might be looking forward fancier reports, I can recommend Telerik's suite of controls for reporting. The report template can be built using anything from report designers like Crystal Reports to plain HTML. The latter is likely the fastest but depends on how you plan on using this.
