Database + 11 modules for Windows local network

We seek a database (prefer MySQL if possible) installed, 11 modules to manipulate the data, 2 printed forms incorporating data plus 1 simple control panel. Once the script is installed, there will be one modification rewrite approx 30-45 days after the client gets to know the script to work out bugs and possibly make the script more efficient. At a later time, there will be additional modules needed, which we will request a separate quote for at that time. We will require the source code to be furnished as well. Specs attached.
