Good day, I have over 15 years of experience in the elearning field and more than 20 years of experience in website design and development. I am interested and can do this project for you. Basically, I can build this website for you, but to complete this bid, I need to know some more details as follows: Do users need to be registered & login to submit the form and create the pdf file? Do you want data of users to be saved in a database to refer to later? Or, just submit the form and create a pdf on the fly. Do questions have conditions related to other questions? In principle Moodle is a great and powerful system, WordPress is another system to rely on as powerful frameworks, but for simple uses, Moodle is not advised as it has ton's of features and somehow heavy, what I suggest a simpler one that could use WordPress, or just a simple bespoke website. Hi, if this project is still open I'd like to apply - I create online surveys and generate pdfs using the results as my primary area of work so I believe I could definitely create a solution to suit you. I mainly use the open source php libraries Limesurvey (to create the questionnaire/survey) and TCPDF to generate the pdf. I create the reports to any design or specification using php scripts, and you can run automatically or manually on your webserver. I can show you examples of the reports and surveys I have created, I have a sample survey which automatically emails a pdf report to the user on completion that I can send you the link to if you're interested.
