MailChimp Webform Integration on Wordpress Site

I have a Wordpress website and I'm looking for a web form expert to create 3 web forms. Each form must integrate with MailChimp. The first webform is a lead capture form on the home page. It should allow users to enter their first name and email address. The user's entry should add them to the MailChimp email marketing list. The second and third forms are basic contact/suggestion forms. These forms should submit the user's name and email to the MailChimp email marketing list, and send me an email with the content that the user submitted. Create a web form with fields for first name and last name. Form submits data to a MailChimp email list. 2. Create a web form for the "Suggest a Topic" page. Form submits data to a MailChimp email list and sends an email to me with the content that was entered. Fields include First Name, Email, Topic of Interest, and Description/Recommendations. Form should have a CAPTCHA field to block spam. 3. Create a web form for the "Contact" page. Form submits data to a MailChimp email list and sends an email to me with the content that was entered. Fields include First Name, Email, Phone Number (optional) and a Content text box. Form should have a CAPTCHA field to block spam. 4. Each form must have very clean formatting that fits the Wordpress Theme. This is a requirement and especially important on the Home page lead capture box. No basic forms with sharp corners and plain text. PROVIDE PREVIOUS EXAMPLES of similar work with your quotation to be considered for this project.
