Laboratory Results Portal

Summary: Build a website to be used by a laboratory to distribute results to both patients and providers in the form of PDF files. The files will be both viewed and downloaded from this site. Once the user logs in they will get the "Results Report" page. This page will have a table with four (4) columns: Date Requested, Patient Name, PDF link, Select. The "PDF" column is a clickable link that will display the pdf in a new tab. The "Select" column will be checkboxes. Required products: VS2017 vb.net, SQL 2014 The site will have three pages: "Start", "Login", "Results" "Start" page will have two buttons to select the login type: Provider Login Patient Login "Login" Page will differ by the selection made on the "Start" page: Provider login requires two (2) items of data: UserName and Password Patient login requires three (3) items of data: One (1) of the following plus Date of Birth and ZipCode. 1) 10 digit mobile phone number 2) email address 3) social security number Example: 10digitphone + DOB + ZipCode A successful login will take the user to the "Results" page "Results" Page: There will be two (2) date pickers, to identify the start and end date for the queries. There will be three (3) buttons: 1) "Find Results", to do the query and populate the data grid. 2) "Select All", to select all of the check boxes in the "Select" column. 3) "Download Selected", to start download of all selected PDFs.
