I have a website for rating and reviewing university courses.

Need a pop up window that allows students to rate and review courses. Here's my (preview) website just so you can get an idea of the colors and theme of the website: www.unabie.com/preview. Look at the login window, this is the kind of elegance I want.

The window should have the following:
Course code and name on top. Use the example: ELEC 252 - Electronics
Then a field for the professor's name saying: Course Instructor:________
Then the ratings criteria:
Course difficulty
Interest level
Course workload
Dependancy on textbook
Overall enjoyment level

The ratings are from: Very low - Low - Moderate - High - Very high

Then comes a small field for writing a review.
