Fix orderform MSSQL ASP

I have the files and database from a other site that we have put onto our website, I have set up the MSSQL database and uploaded the files, part of the order works but there are a few things that need fixing. This should be a simple quick fix to get the site working. I have ftp and MSSQL web administrator details, SQL server 2000.

Attached is a zip file with the URL to look at and the order files.

the error on page 2 is

Microsoft OLE DB Provider for SQL Server error '80040e2f'

Cannot insert the value NULL into column 'DATE_ENTERED', table 'WS110518.dbo.ORD_ORDER'; column does not allow nulls. INSERT fails.
