Art Approval Form

I need a customizable art approval form. The form is a pdf which is always viewed in LANDSCAPE FORMAT. I need to be able to choose and change information in the header, Job Information, Print colors, Material, Approval Box and Special Instructions. Or have the ability to type in new information for new customers that aren't in the pull down menu. Once I type this information in then its saved where I can access for repeat orders.
The art in the center will change for each art approval in image, size and finishing details. I need the ability to drop in pdfs or .ai files and have the art scale to the appropriate size in the middle of the page.
See the attached document for the basic idea. Any suggestions or ideas you have to make this more polished looking or easy to modify is appreciated.
