Bookmarklet needed - Urgent

Bookmarklet needed with two buttons/functions : 1) a 'submit' function to capture 10-15 fields (name, address etc.) off popular e-commerce order admin pages and securely transmit via existing API into SQL database, and 2) a 'search' function which captures the same 10-15 fields so that they can be run through a search of the same database with a return high/medium/low 'alert_level' variable and a URL link to a page with more detail.
