MS SQL Bulk Insert with fmt file

I have an ASCII file with fixed field lengths and need appropriate table constructed to import this data with BULK INSERT statement into a SQL 2005 database. You will be supplied with the data file and documentation outlining where each column starts/ends.

Your deliverable should be script for a sql 2005 database with one table in which this data is imported. And script for a stored procedure which when called will use BULK INSERT to import all the data into the table. You will also probably need to define an fmt file.
