HTML5 Signature App to work on iPad and Android Tablets

Hello. I require a Signature app in HTML5, this is to be placed on a standard PHP Web Page, then if signed and submitted, it would save the signature graphic to the server as a JPG or PNG file.
This must work on Android and iPads/iPhones.
I have found 2 examples online of what to replicate (see below). All code you submit must be original, not copied and we will retain all copyright of this and ownership.
