154881 Database search form

I want a simple script where the user can search for one of four fields, either enter Container Number, Bill of Ladding, Airway Bill or Tracking code.

Once they enter they will be taken to a page where they can find additional information on the goods posted. Any free script customised to do this will do for example: [url removed, login to view]

I am on a VERY tight budget and the only fctor will be the cheapest bid.
