Create MySQL DB and load script from CSV File

I have a CSV file that contains approximately 320 fields and over 3 million records.
I need a script to import the data into a MySQL database, as well as the table/structure created.
First line of CSV file has field names, they need to be converted to appropriate database field types
## Deliverables
I will provide the first 1000 or so records so you can see what the field data looks like. ? Please pay attention to this when creating the database layout so that fields are created appropriately (i.e. no VARCHR(200) for everything).
The data will be deleted/refreshed monthly. ? Database is MySQL 5.x
Ideally, I would like some web-based scripts (php) to search the data.
