Spiritual Gifts for IWeb Site

I have had a spiritual gifts test done through rent a coder for a windows front page site and have switched to iweb. ? I would like to spiritual gifts test that has 120 questions where they can answer from 1-5 to automatically send the info to a database that will add the scores and come up with the top three gifts based upon the highest score. ? I would like a copy sent to me and a copy sent to the person. ? I would like to make this a passworded entry so I can capture some information from the person filling out the spiritual gifts test. ? In summary here is what I need

1. Entry form for name, email address, how did they here about our site

2. Spiritual Gifts Test where they can take online

3. Ability for the forms to automatically add up highest scores on all and send top three to the person and one to us, with all scores.

I have some code that I can send from the windows application but need this to be on our ministry website at [url removed, login to view] and ministryworks.net.

Should be a simple and fun project for a programmer who can program for a MAC.
