Need attachment added to my contact form

I need an attachment added to my contact form - I have the interface set up and the contact form works how I have it I just need the attachment coding added to the php file - I have enclosed the php file here
I need the max file size to be 10mb
Allowed extensions jpg, jpeg, gif, bmp, png, pdf (if you can think of any other common ones I may need add)
