Web Application development for Barcode generator

Dear Freelancer,
We request a web application to manage documents related to a business process. Fields data is capture in the web application from users and this data is used to generate barcodes. This barcodes are inserted/printed in documents and then user can print the documents from the application.
A mockup and application description is attached. We need to develop an application for the following purpose:
- multi customer and multi department
- easy to configure, admin users can create and modify business process documents and forms
- easy to use, one page to interact with the process
- security based on users and groups
- web application with responsive design to be used on PC and tablets
- menus, messages an rest of UI content needs to be translated into spanish.
What we expect to receive:
- full functional web application to be deployed in our server
- source code and comments of the web application for further improvement and modification
- use of current common technology and programming languages with a light architecture PHP, SQL Server, MySQL, Open LDAP, Microsoft Active Directory.
