Yii2/PHP/MySql

Summary:

Add a module/page to perform CRUD operations. Page will contain two tabs with fields shown in the attached imaged. Existing site is built using PHP/MySQL/Yii2 site.

Detailed scope:

Step1: User existing Site:

Clone following open source project. A page/module will be added to this site.

Step2: Add a new side menu link

Step3: Build schema

Based on the fields shown in attached images, build tables/schema.

Step3: Build the page.

Fields needed are shown in attached image.

Details of data types of each field will be provided in an excel sheet.
