SQL query to identify Name/address duplicate suspects in a table

We are looking for an efficient query to identify possible duplicate contact records in a ms sql 2008 table.
Table structure is:
Firstname
Lastname
Company
Address1
Address2
City
State
Zip
Workphone
Homephone
Cellphone
The challenging part is fuzzy and soundex searching on name and address fields. The result list will be displayed by a web app to a user to confirm duplicates and deal with them so it doesn't need to be perfect, but just go beyond basic exact matches.
We'll work with you to refine the tolerance. We'd prefer a bid from someone who already has a similar query/algorithm developed who can adapt it to this table.
