Transfer website to new host (URGENT)

I was just notified that my website's hosting company will shut down later this week. I need to transfer the website to another host by March 8. I have another shared hosting account that the site needs to be transferred to.
The website includes a blog running on WordPress version 1.5.1.3 (very old). The WordPress configuration, add-ons, themes and database must be transferred to the new host and upgraded to the current version of WordPress (v2.1.2).
This project must be completed by March 8. The project will be complete when the website is running and fully functional with the new host and the current version of WordPress. All e-mail files, logs, database files, etc. must be transferred to the new host.
I'm looking for someone with expertise in website hosting setup/transfers as well as WordPress experience.
Please provide the steps that you will take to complete this project. Thank you.
