Custom form created within Wordpress Environment

Hello,

I am looking for a professionally created form developed in WordPress that will work much like a mini-survey would. The form would ask specific questions using clickable input fields. Once all the informstion has been inputted, the form will automatically calculate the best plan tailored to them. The form would also allow the customer to create their own custom plan, giving them the ability to pick between various features, resources in a way that saves them money and is specific to the service they require.

The form itself would work like this:

1. Have specific questions for the customer to input

2. Click the 'Calculate Plan'

3. After a few seconds generate a result - Showing a specific plan appropriate for their needs in accordance to the answers given.

4. Offer an alternative button to 'Create a personal plan' where they can add features and resources for a more specific and custom basis for the customer.

5. Once the custom plan of features are added, show the breakdown of features and services with a cost breakdown for the checkout.

6. Add the generated plan or specific plan to the basket ready to be paid for.

As the website is currently running on WordPress, custom code may be required to create the speicifc form in the scope of this project propsal.

I have images showing the design of the form that i believe would work well with the project, in addition, these images would be sent to you via a DM in the event that the freelancer DMs me first.

I will NOT be accepting automated responses for this project as I am looking for Freelancer's who will be specific and understanding the scope of the project. We urge you to ask any questions if you wish regarding the project and in doing so I will respond appropriately.

Estimated budget is between $10.00-$20.00, however we can push between $30.00 - $70.00, but no higher, so do keep this in mind before bidding for this project. I understand this is a tight budget, but considering COVID-19, it's important I allocate resources elsewhere as well.
