Need survey created and payment processed first

We are in the process of developing a simple online study program. Essentially the site will offer a testing program which is about 10-20 questions with multiple choice answers. Here is a sample: [url removed, login to view] To view the testing program (course content) click on Member Sign In on the home page. Password is capt1 Our objective is to keep things simple. The student should be able to access the available program page, get a brief description of the program and then have the option of purchasing it (via paypal). Once the student purchases the test program they can then take the test on line. As far as the test results go, we want to offer a test with an evaluation format as opposed to just a multiple choice test with a review of the answers. We also feel that it is necessary to set up the answer keys or test evaluation keys so that they can not be printed easily (if possible). The test program will consist of approximately 3 multiple choice tests with Questions and answers- 1 oral based fire scenario consisting of a fire scenario with questions and a written answer key- 1 oral based supervisory scenario consisting of supervisory scenario, questions and answer key. We do not need a data base, do not need scores sent to us. We don't care about the test results, that is for the students only (we don't need it compiled somewhere).

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
