Customer Service - Patient Panel Management

Customer service freelancer to schedule patients for our dietitians and create efficient driving routes. Must be able to make 210 outbound calls a day and schedule 70 patients. Must have advanced excel skills and be able to use multiple software packages Must be fluent in English and have excellent verbal communication skills. Must have a computer with dual screens. Dietitians at Home is a diabetic patient management company. Our mission is to improve the lives of our clients by supporting them in changing their lifestyle. Dietitians at Home's Coordinator is responsible for coordinating the logistics aspects of our program to ensure that all of our clients are able to receive the services they need timely. Our team coordinates the schedules of our registered dietitians, ensuring that our clients are seen timely and our dietitians have an efficient schedule. Our team is responsible for communicating with clients in a motivational way that allows our clients to feel empowered and supported. Our Coordintor makes extensive use of the following tools: Microsoft Excel, pivot tables and data analysis Scheduling and routing software Electronic medical records Automated client contact systems Clinical data analysis We are looking for candidates with the following characteristics: Desire to take personal ownership for the clinical outcomes of our clients Ability to communicate with a broad mix of individuals, including our dietitians, our clients, our clients' family member and other members of our clients' clinical care teams. Ability to collaborate and work as a team with individuals with diverse backgrounds and skill sets in order to ensure the highest level of care for our clients Highly organized with excellent project management skills Creative thinker who consistently looks for, and develops new ideas to improve our processes and better communicate with our clients
