HTML form data to Google Sheets & form upload to Google Drive

I am looking for a developer with experience working with Google Sheets, Google Drive, JavaScript and / or PHP. Basically, what I am hoping to achieve is a simple html form with a few input text fields & a file upload option. I would like the input fields to populate a google sheet & the file upload to be sent to a folder on google drive. The sheet should store the input field’s data, but also a url to the uploaded file. I have come across multiple tutorials and scripts online about how to achieve this – I just don’t have the time to complete it. The form processing can be done in JavaScript or server side PHP (the webpages are pure HTML). The post / get requests can be carried out using JQuery / ajax as it is already available in the project. The fetch api would be the preferred method. The usual form validation will be required along with file validation ie. (only allow .pdf, .doc, etc!) I can provide HTML, JavaScript, PHP if required, or if you would prefer to build from scratch or an existing template, that is fine also. Preferably, the freelancer would have experience with everything listed above. I can provide support throughout the project to help move the project along.
