app for making home inspection reports

home inspector, I want an app that will allow me to take photos and insert them into a report, with comments. Need boiler plate to add and save comments. check box to add comment. need ability to send report pdf via e mail after payment. need to coordinate calendar. from website or scheduling inside app. The report is simple home inspection. it has lots of photos, need to be downsized. The report will have several template forms. General home inspection, Pools, Termite, Radon. Multi, commercial. Need Realtor contact list and simple one click notifications. Small start up business. As I will take over 100 photos per inspection it is desired to have a simple few clicks as possible, for each photo. A simple way to select a boiler plate comment, or add a voice or text to each photo. Will be using galaxy smart phone most likely. No advertising, No continuing fees. I must be able to upload a logo.
