Build a form and save data to pdf

hi i need a form built on my website [login to view URL] form-design is attached. 1: i will provide my T&C's you put on box (READABLE) - user can not change it. 2: then there will be checkbox to Accept T&C's 3: User will select Signature style 3.1: User will enter FULL NAME 3.2: User will enter INITIALS 3.3: User will select signature style (dropdown should populate automatically with different font-styles where user type full name) and it will show on page as per provided design. like this [login to view URL] but as dropdown 4: a checkbox for signature representation when form will submit. it will create PDF that will contain all form data and save on hosting path something like this public_html/pdf/[fullname]-[datetime].pdf and show a thankyou message on web page.
