Create Jquery form

Hello, I need to create a register form. The visitor will choose a category (tennis, basket, football) (radio only one choice possible) Then a division (classic, junior, senior) drop-down input (0,1,2,3,4) Once the visitor selectioned how much people he want to register to each categories (i.e 1 classic, 2 junior, 1 senior) The page will display the same number of text fields with matching label i.e junior [___________] classic [___________] classic [___________] senior [___________] The script will generate inputs of hidden type with values like this <input type="hidden" value="{{label}}" name="generated_field1"> The maximum people a visitor can register in the same form is 4 if a visitor selects (classic 2, junior 3) the form will reset and a red message will be displayed "you can only register 4 people at a time" and resets the values lastly, each category has a price (in a hidden field), the total price must be displayed Thank you for your interest
