Create animation sample page

I need a single form page to show sample animations, VERY similar to this: [login to view URL] Except it will use the animations from this library: [login to view URL] ^^ Please look at the aos library carefully The form will have 5 fields that let the user choose the animation options. On submitting they will see the animation happen with the options they had chosen: These are the fields: 1. A select box with the animation to choose from: example: fade-in 2. Another select box for data duration (1 - 10). This corresponds with the aos library feature: example: data-aos-duration="3000" 3. A select box to choose the data offset (1 - 10): This corresponds with the AOS feature: example: data-aos-offset="500" 4. Another select box to choose the easing method: example: data-aos-easing="ease-in-back" When submitted, an element will animate below showing exactly what the animation looks like with the library. -- this should happen without page refresh, so it would be through AJAX. The element that animates below can just be a rectangle with text inside of it. No javascript libraries are allowed other than jQuery. No CSS frameworks like bootstrap, and no PHP frameworks please.
