I require a set of webforms designing to attach to an access database. In the future we will attach it to a SQL Server database.
I have attached a word documrent of the forms I need.
It consists of three forms - 1. List view - 2. New & 3. Edit
There will be no need for the Add new form as we will be pre-populating the database with records.
The flow will be -
View the list - select a system - goto edit form.
The major addition to the attached forms is we need to collect Risk and Consequence data for a 6 year period - the current year and the next 5 years.
I would suggest a tab at the Risk assessment stage.
I have the access database table structure already developed and will supply this to the successfull coder.
Only top coders need apply for this job - I have a strict deadline that has to be enforced - I will be selecting a coder within 48 hrs - the successfull coder must complete the forms within 7 days.
I would like you to copy the layout of the current forms but where orange is the predominant color - I would like to use blue.
The first list view form must be alternate shading rows with on hover shading as well.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
