HTML FORMS, AN IMAGE MAP WITH LINKS, ARRANGE WEB PAGE

Hi, I have a project that I needed to get done as soon as possible or within 2 days...since I still have to study the codes. My project is a Booking Form form used by travel agency, when users send the form in PHP format.. should write into a file using in a PHP code then lists all the number of travelers and their flight destination. Additionally, I need an image map of my webpages where their is a hot zone so users can get the information about my web page by just clicking the hot zone. I also need to arrange somehow the position of my webpage. I have the sample codes that you can use to generate my requirements, but the rest will be your codes. I will provide form layout and some codes that you can use..its up to you use it or not or use your codes. Codes must be understandable by beginners in programming like me. If I can't understand the code you use, you have to change it. I need it to be working codes. You must validate the form, javascript code, html, and CSS. You must have local server to test the php code, at least a photoshop or any image editor to create a hot zone point. I have the sample code for this for you to use.
