15 item order form cart

We have a client who needs a shopping cart form WITHOUT the commerce functionality. At the end of the shopping experince NO CREDIT CARD information will be collected. The order will just go to an email address. There will be approximately 15 print products. Each product will have at least 5 values. Some will have more than 5 values. Here's the way a simple business card order might be processed. The user would select "Business Card" from a list of products on the home page. That link would take them to the Business Card page where they would see a sample picture of a business card. There would be a form on the page. The user would work their way down the form, filling in; Name Address Phone Mobile email For each entry the data would be captured in the form AND displayed somewhere on the page, (maybe where the business card sample sits) so the visitor can see how "their" business card will appear in its final version. So when they enter their Name, for example, the words John Doe would be replaced by the user's name. Then the user can pick: ink color (from a drop down) quantity (also from a drop down) When they're finished the business card has all the user's information in it. Then they can add this business card order to their shopping cart AND select another product, if they wish. Like maybe letterhead stationary, or a poster. Then when they are finished they can click on the checkout link. This will calculate their total order, (the products will have quantity pricing.) The checkout screen will show all the products they've ordered. Then they can enter billing and shipping information. We would like to give the user the option of having seperate "ship to" addresses for each product in case one buyer is purchasing for multiple users that work in different locations. The user would then hit submit and the order would go to an email address. Additionally, it would be nice to offer the user a printed copy of what they ordered so they could match it against the final product. For instance, a proof of their business card so that when the printer sends them the final proof they can match the two documents to make sure the user entered the correct information and that the printer is prepared to print the correct information . Having said all this, the main functionality of the site is the entry of the data itself for each product, so it the dynamic "viewing" of the final product isn't possible we'll settle for simple order entry.
